The Library is an official Passport Application Facility. U. S. Passport applications are accepted on a first come, first served basis.
1St Time Applicants/Lost or Stolen Passports/ Children's Renewals
(Bring a completed DS-11 Passport Application Form)
Please also bring
A recent Passport Photo-2X2
Valid Driver's License, State Identification Card, Employee ID or Military ID
Your original Birth Certificate or Naturalization Certificate (Certificate will be returned to address on your DS-11 application)
Check or money order (NO CASH) made out to the U.S. Department of State will be accepted in the amount of $110.00 for adults and $80.00 for children, and a $25.00 fee by check or money order made out to Norwalk Public Library.
